Your Impact in This Role: Lead end-to-end case management and coordinate personalised rehabilitation services that empower clients through every stage of recovery. Support individuals in regaining their functional independence , facilitating successful return-to-work outcomes through tailored interventions. Conduct comprehensive workplace and functional assessments to identify needs, capabilities, and potential challenges. Uncover and address barriers to return-to-work (RTW) by applying a proactive, solutions-focused approach. Design and implement customised rehabilitation plans that promote sustainable workplace reintegration for injured workers. Collaborate effectively with key stakeholders , offering clear, practical recommendations and facilitating positive, goal-oriented outcomes.
